 Typically, enzymes in your intestines break down fat from food so your body can absorb the fat. But the active ingredient in alli attaches to some of these enzymes and prevents them from digesting about a quarter of the fat you eat. Because undigested fat can't be absorbed, it passes out of your body instead of turning into calories.
 The main side effect occurs when you eat a meal with too much fat while taking alli. If so much fat is blocked that your stool can't absorb it, you might have side effects. Because they are not harmful, we call them "treatment effects." These include loose or more frequent stools, an urgent need to go to the bathroom, or gas with an oily discharge. The excess fat that passes out of your body is in no way harmful. In fact, you may recognize it as something that looks like the oil on top of a pizza. Treatment effects can be lessened if you stick to reduced-calorie, low-fat meals that average 15 grams of fat per meal.
 - Don't use alli if you are pregnant or breastfeeding, if you have had an organ transplant, or if you are taking cyclosporine.
- Do not take alli if you are allergic to any of the ingredients in alli capsules.

After having used Xenical a couple of years ago, i tried alli for several months when it first came out, and again after about a year later. I found that each time, I lost about 15 pounds (while doubling the dose to equal the prescription strength) without changing my diet. The side effects became predictable and therefore manageable, and I soon found myself eating a little more of "the good stuff (fat)" because I knew that it would be whisked away. I got tired of spending my money on alli each time, and after stopping alli, I gradually gained the weight back.
If you need a kick in the pants to cut down on fat in your diet, this may do it for you. However, if you restrict fat in your diet, you WILL loose weight without alli. If you want to continue to eat what you want and take alli, you will loose weight as long as you continue to buy the product. I think that it was a mental thing for me, knowing I could eat more and keep my weight down. From personal experience with this product, the more fat you consume, the more you will excrete. I guess I was just looking for the most bang (literally) for my buck.
Even if you follow a low fat plan and loose with alli, I just don't see how you will keep it off unless you further restrict your calories when you discontinue buying and taking the product.

I was hesitant at first. Bought this in November, 2009 and didnt start using until Feb 2010!! I only tak eone when Im having a meal with fat. I read the Alli website carefully and signed up foor thir support. I have not had any of th side effects, maybe because Im not taking it every day.

this is a good product BUT you have to follow the instructions to the "T"! no messing around or your body will react in a negative manner which is fully instructed in the starter pak. i have been on the program for 3 weeks and have already lost 7lbs. highly recommended.

*Based upon the reviews of this product, I suspect I'm not the typical Alli user, so my results may be different than normal; YMMV.
Six months ago I began a low-carb (<75g/day), low-fat (<75g/day) diet with an average caloric intake between 1,200 & 1,500 calories/day; I also began running 2.8-4 miles per day, seven days a week. I'm 5'9" and weighed ~190lbs, with a BMI of 28.1 (overweight); my systolic blood pressure was a bit high, my diastolic fine. After five months of this routine, I found myself at ~170lbs and both systolic & diastolic were now great. However, I had plateaued at that ~170lbs...even with changes to my diet (lowered my carb/fat intake even further) and varying my exercise routine with more sprints, hilly terrain & longer runs. My BMI was still considered "overweight" (but just barely) at 25.2.
So I began doing a bit of research, and though I really wanted to steer clear of weight loss drugs, I stumbled onto Alli and the thousands of reviews of it on the internet. My initial concerns, as they seem to be with everyone, were the "treatment effects" and what seems to be a relatively low reduction in fat intake (25%). But, as I was already on a lower-fat diet, I decided to give Alli a try for one month...just to see if it would have any impact and help me drop the final 7-10 pounds I felt I needed to lose.
Well, it worked. While maintaining the same dietary intake & exercise routine I mentioned earlier, I was able to get down to 161lbs by the last Alli tablet (90-count) in the bottle. The fact that I was already eating a pretty healthy diet also meant I didn't experience a single "treatment effect" throughout the entire process. Mind you, I do eat "bad" things from time-to-time, things like 12oz glasses of 2% milk, chocolate cake, a cheeseburger (though no french fries), a small DQ blizzard, etc.; I'm not starving by any means. But I'm very sure that having already adjusted my diet for several months prior to using Alli was a major reason why I didn't experience any "horror stories."
As I said I previously, my experience with Alli may be atypical since I had already made the "major" two life changes (diet & exercise) prior to starting with the drug. If you are really overweight or even obese, you may want to save yourself some money and make those changes (turn them into habits) prior to using Alli. Limiting your fat/carb/calorie intake while exercising will definitely cause significant, healthy weight loss; Alli is not the long-term answer, only a short-term tool to help you when you've "peaked" on your diet & exercise. However, I guess the experience of a "treatment effect" may be the motivator some people need to stick with the change in diet. At the end of the day though, it's not rocket science (or in this case, chemical engineering): You must burn more calories than you intake, and the only way to do that is to eat healthier and exercise.
I'm giving Alli four-stars, as though it worked as advertised and I didn't have any problems taking the drug, I can see where it could be expensive if you were to use it long-term. But now that I've reached my weight & BMI goals, I'm no longer using it; the changes in my diet & exercise have become long-term habits and should allow me to maintain my new weight without problem.

I tried this for 2 months, and feel like I only had results from doing a regular diet and exercise and not significant results from using alli. It wasn't worth all of the money it cost me, the torment it put my body through, with the greasy discharge, messy toilet orange oil marks, anxiety when going out in public, and having to take a multivitamin at night instead of whenever I want. I didn't think this was going to be a dream pill and solve all of my weight problems, but for all that mess it wasn't worth it! The alli program wanted me to eat 1200 cals a day and under 12 g fat per day. It was hard and I lost some weight, but I'm not giving the credit to alli, I'm giving it to myself. In my opinion if I were to do the same diet & exercise without alli, I would have lost the same amount of weight or had a difference of something minimal like a half a pound.
This program was too strict for me, and not going to work for me long term. I'm a busy stay at home mom runnin' after 2 little kids, and can't afford to spend all of this money and time worrying about calorie counting and taking pills with every meal, and vitamins at night instead of in the morning like my usual routine. I gave it a shot for 2 months, I'm not satisfied with the results and now looking into the Atkins diet as something that will work for me long term. I don't like the fact that it keeps my body from absorbing some nutrients and vitamins. I want all the nutrients and vitamins I can get out of my food, not just some artificial multivitamin.
I had treatment effects for 2 weeks, then it all settled down, but was still in the back of my mind and had occassional "effects" throughout my 2 month alli adventure. On the plus side the website does have an excellent support system with regular supportive emails, and weekly weight check in's and was a great source for recipes and help.
Best of luck to anyone who tries alli, but this was my experience and it did not work for me.
